Discharge Planning

HUHS Case Managers work with attending clinicians, patients and families during the course of a hospital admission to identify potential discharge needs.  Once identified, the department will make all necessary arrangements through HUHS preferred providers, update your Primary Care Physician on clinical progress, and assist you through the discharge process.  Once home care services are initiated, your Case Manager will stay in close contact to ensure that your needs are met.
