Social Services

The Care Coordination Social Worker provides assistance to patients and families in identifying and addressing psychosocial and environmental concerns.  She or he collaborates with patients, family members, and health care providers to determine the most appropriate options for clinical physical and mental health care.  In addition, the Social Worker provides Advance Directive education and follow-up.
