Immunization Forms

Compliance with Massachusetts Immunization Law is an enrollment requirement for all Harvard University students. In order to register for courses, students must provide a complete immunization history by the deadlines shown below. Students are encouraged to review the requirements promptly to allow time to obtain and submit documentation.

Deadlines
Fall Term Students: July 15, 2008
Spring Term Students: January 16, 2009

Incomplete or overdue forms may delay registration

Note: Hepatitis B immunizations require a series of three injections over a six-month period. Students who are not yet immunized against Hepatitis B are expected to complete at least two injections prior to registration.
