  • The time to think and talk about health care preferences and to establish an Advance Directive is before you need to depend on it. HUHS is interested in educating patients about this important subject and in providing information to help you make meaningful choices. Discussing your thoughts with family and with health care providers can help with your decisions.
  • You are invited to view a ten minute video about Advance Directives at a monthly information session which is held the first Monday of each month at 10:30 A.M. Please, call Social Services at 495-7872 for meeting location. This session is a good opportunity to address your issues, obtain forms, or to just learn more about this subject.
  • You may also contact Social Services for individual assistance with your questions or to complete forms by calling 495-7872 or emailing CareCoordination@huhs.harvard.edu If you wish, family members or the person you are considering as your health care proxy may accompany you.
  • Your clinician is willing to discuss all questions about your condition or treatment decisions. Be sure to arrange an appointment time for this discussion and come prepared with questions.
  • The forms can be requested from your health care team. You can also visit the American Hospital Association for additional information and resources.
