Where Is the new practice be located?

The new practice is located in the same location as the HUHS practice, at the main HUHS clinic in the Smith Campus Center, conveniently located in Harvard Square.

What are the New practice's hours?

The hours remain the same, Monday-Friday 8:30am-5:00pm.

Was I automatically transferred to the new practice?

No. You will need to register as a patient and we will need your permission to transfer your medical records.

How do I become a patient of the new practice?

  1. Contact Atrius Health Patient Registration at (800) 249-1767 (Monday-Friday, 7:30am to 6:00pm) to register as a new patient and select a pediatrician
  2. Complete the Medical Record Release of information form to authorize HUHS to provide your medical record to Atrius Health and return completed forms to Courtney Applegate (HUHS 6th Floor, 75 Mt. Auburn Street, Cambridge, MA 02138)
  3. Inform your health plan of your pediatrician selection – if you are a HUGHP member, please contact HUGHP Member Services at (617) 495-2008 or mservices@huhs.harvard.edu

Where do I get the Medical Record Release of Information form?

You can either download it, pick it up at HUHS, or we can mail it to you.

Does each patient need a Medical Record Release of Information form?

Yes, each patient will need to complete a Medical Record Release of Information form. Patients who are 18 years or older must sign their own release form.

Where do I return the Medical Record Release of Information form?

Return completed Medical Record Release of Information forms to Courtney Applegate – HUHS 6th Floor, 75 Mt. Auburn Street, Cambridge, MA 02138. HUHS will pass this information to Harvard Vanguard for record transfer.

Do I need to switch my insurance plan in order to visit the new practice?

No. Anyone who is a member of a health plan accepted by Atrius Health (Harvard Vanguard), which includes HUGHP and HUSHP, can become a patient of the Harvard Vanguard Pediatrics at HUHS practice.

What will happen to my referrals?

Clinicians in the Harvard Vanguard Pediatrics at HUHS practice will typically refer patients to clinicians in the Atrius Health network, unless a referral to an HUHS clinician is deemed necessary for continuity of care by the referring physician.
