Medical Records and Immunizations
The HUHS Health Information Services/Medical Records department is responsible for safeguarding the medical record information of each patient. We also process your health documentation, immunization forms and requirements, and authorize access to your online medical record.
Hours of Operation:
Monday-Friday: 9:00am – 12:00pm and 1:00pm – 5:00pm (closed daily from 12:00pm – 1:00pm)

Immunization requirements for students are based on the Massachusetts Department of Public Health (MDPH) regulations for school entry, as well as additional institutional requirements where appropriate for our campus community.
Harvard follows the Commonwealth of Massachusetts immunization requirements for all students. In instances where national recommendations and Massachusetts law differ, Harvard will follow Massachusetts state law.
