Care Coordination

Care Coordination’s Social Work Case Managers are available to help students, employees, and retirees who receive their primary care at HUHS. We can support you in making informed decisions about your medical, social, emotional, insurance, and vocational issues related to injury or illness. Our goal is to assist you in navigating the health care system and in getting the care you need. We do not make decisions for you but rather help give you the tools you need to do so.

What does Care Coordination do?

Follows our medically hospitalized patients throughout their stay and calls when you are home.
Works with those who have complex medical care needs.

Connects you with community resources, when applicable.
Educates about Advance Directives* (Health Care Proxy and Living Will Wishes).

Health Care Proxy
- Identify someone you know and trust to make health care decisions for you if you become unable to make those decisions.
Living Will Wishes
- Provide specific written instructions for your future medical care.
